Leaving Care and After Care Support Service

WHAT CAN THE PROGRAM OFFER?

 

The aims of the leaving care and after care support program are:

  • To support and work with young people in developing the necessary living skills to successfully achieve the transition from care to independent living
  • To provide ongoing support to young people and their families for those who return home after leaving care
  • To strengthen the links between the young person and their family
  • To provide young people, prior to leaving their ‘out of home care' placement, the opportunities to experience independent living situations
  • To work with statutory services in developing long term after care plans
  • To work with key services in providing a comprehensive support plan for young people who have left care
  • To provide links to educational and vocational opportunities
  • To enable access to secure and safe housing after leaving care
  • To enable young people who have left care to maintain their links with significant adults
  • To provide practical support to young people who have left care
 

The Leaving Care and After Care Support Service can provide case management and support, which will not be time limited. It is planned that the service can be available to these young people up to their early adulthood years.

WHO IS IT FOR?

The target group is young people (16-18 years) who are leaving care in the Loddon Campaspe sub-region, with a particular focus on those who have:

  • Limited options for housing.
  • Limited support from family and other community supports
  • Priority will be given to young people who have been in St Lukes Out-of-Home care.
