These services provide individual and group support, activities, socialising, and housing related rehabilitation and support for people aged between 16 and 65 years with mental health issues.

Psycho-social education and rehabilitation group

For people with a diagnosed mental illness experiencing social isolation this group supports and re-enables through participation in social, recreational and psychosocial educational activities. It also assists participants to improve their quality of life, increases their independence in the community and supports recovery by increasing community participation, building self esteem and confidence.

Housing and Accommodation Support

St. Lukes Deniliquin is the provider of the Housing and Accommodation Support Initiative (HASI) program for the lower western region of NSW. HASI aims to improve housing stability and community participation for people with a mental illness. In partnership with Deniliquin Community Mental Health, St Luke’s aims to assist individuals to live in the community and maintain accommodation with support. The program facilitates access to accommodation, provides flexible home based outreach, identifies goals and develops individual programs for social and living skills whilst focusing on people’s strengths and increasing community connectedness. The program complements clinical and community services.

Recovery Focused Mental Health Home based outreach

Home based outreach services provide individual support and rehabilitation to people experiencing a disability, as a result of a mental illness, in their own home though strength based philosophy. St Luke’s works in partnership with people to identify their goals and develop support plans that promote choice and decision in their own journey of recovery.
