When referrals have
originated in Child FIRST, it is the responsibility of Child FIRST
and the Team Leader of the allocated family support program to
ensure a smooth handover process occurs.
The key tasks for
Child FIRST include:

Informing the family of the allocation
outcome

Clarifying the next contact with the
family and considering the relative value of a face to face
introduction to the allocated worker. Generally this process is
adopted when the Child FIRST team have had a significant level of
contact with the family, provided a response role or significant
active holding role.

Conducting the handover meeting and
providing further information to the allocated Family worker, with
the presence of the family. Key information provided includes
further assessment information relating to the families challenges,
coping skills, strengths, resources and key support
networks.

Handover of duplicate copies of
referral and other relevant information that has been recorded by
Child FIRST and able to be released (excludes reporter/referrer
details when know and protected).
