The Personal Helpers and Mentors (PHaMs) Program is  designed to assist and support people who are experiencing symptoms and functional limitation as a result of serious mental illness.  Government funded, PHaMs is a community- based psychosocial recovery focused program, which provides people with the one-on-one support, opportunities, and services that will promote wellness and reconnection with their community.  People who join the program will partner with their Case Worker who will support them in their recovery journey.

he Case Worker aims to empower, encourage and support the person to become more independent, confident, resilient and better able to manage the effects of serious mental illness on their life. 

To be eligible for the program a person must be 16 years or older,  no formal diagnosis of mental illness is required.  The person can self-refer or have a GP, family member/friend contact the program if they wish.  Those eligible experience significant difficulty coping with their mental illness, and may find day-to-day living, self-care, and socializing is very challenging.  The program covers specific postcodes, (see” program postcodes”) and information about the program or referral can be accessed through the Program Team Leader at St Luke’s Castlemaine – ph:  (03)  5470 6266.
